The Home and Community Care Program for Younger People (HACC PYP) supports Victorians aged under 65 and Aboriginal people aged under 50 in optimising their health and wellbeing, while helping them maintain or regain their independence to live safely in their own homes.

In addition, the program enables individuals to actively engage in their communities. These services are available at various locations across metropolitan Melbourne and regional Victoria.

The HACC PYP services are aimed at enabling ongoing community access and connectedness.

ACCS is approved provider for HACC PYP in the following LGAs:

  • City of Brimbank
  • City of Casey
  • City of Dandenong
  • City of Whittlesea & Hume
  • Hobson’s Bay & Maribyrnong
